The cost of not reporting
Failing to report accidents and near misses has real consequences. Underlying hazards go unaddressed, meaning the same event can happen again, potentially with worse results the next time.
The accident book
Every workplace should keep an accident book (such as the HSE's approved form BI 510) to record details of any accident, regardless of severity. This includes the date, time and place of the accident, details of the injured person, what happened, and any witnesses.
